## What the book argues

That the recurring failure of capable organisations is structural rather than a matter of stupidity or bias. Every organisation runs on assumptions about how its world works, and those assumptions decide what gets measured, who gets heard and which warnings are taken seriously. When they stop being right, the people closest to the problem usually know first, and their alarms are absorbed into the prevailing model and returned as reassurance.

## The case studies

Five, spanning intelligence, finance, technology, industrial safety and national security. The October 7 intelligence failure, the 2008 financial crisis, the collapse of Nokia, Long-Term Capital Management, and Deepwater Horizon. The point of the range is that the pattern is indifferent to sector.

## The remedy it proposes

A discipline rather than an exhortation: force every strategic question through twelve competing analytical perspectives and treat their disagreement as the finding. The methodology is implemented as software at Vektora, and the supporting source notes are published separately.
